To assist families the Modbury Hospital Foundation has available specially prepared memorial gift envelopes for use or distribution at funerals. Please call us on 08 8161 2626 to order envelopes to be delivered to a local funeral home.

Your gift will be an ongoing tribute to someone you love, and will assist the Foundation in supporting the Hospital.  Your donation will be acknowledged with a tax deductible receipt, and the family will be notified of your generosity
